Every patient at the Pelvic Health Institute of Illinois has the rights described below.
You have the right to considerate, respectful care at all times and under all circumstances, regardless of age, race, color, national origin, ethnicity, religion, gender identity, sexual orientation, source of payment, or any health condition.
You have the right to know the name and clinical role of every person involved in your care. You have the right to receive information about your diagnosis, prognosis, and treatment options in language you understand, and to ask any questions you have.
You have the right to participate in decisions about your care, including the right to refuse any treatment, to seek a second opinion, and to discontinue care at any time.
You have the right to privacy during your visit and to confidentiality of all communications and records related to your care, as described in our Notice of Privacy Practices.
You have the right to access, review, and request a copy of your medical records.
You have the right to voice complaints or concerns about your care without fear of any consequence. We take all feedback seriously and respond within five business days.
Beyond what is required by regulation, we make this commitment to every PHII patient: you will leave every visit with a written plan, every option explained in plain language, and a clear next step — never a list of referrals to chase down.
