For Referring Providers

A referral portal built for your office.

Direct access to Dr. Johnson. Condition-specific criteria. Pre-referral workup guidance. A dedicated coordinator. And a written communication commitment back to you after every visit.

Our Commitments to Referring Practices

Four promises we keep, every referral.

I.

24-hour confirmation

Every referral acknowledged in writing within one business day. Every time, every condition.

II.

Five-day initial visit

Most new-patient appointments scheduled within five business days. Urgent cases triaged the same day.

III.

Provider-to-provider line

A direct path for clinical questions before referral — not a triage queue. Dr. Johnson takes the call or returns it the same day.

IV.

Closed-loop reporting

A written summary back to you after every visit. Plan, recommendations, follow-up timeline. Your patient does not disappear into our practice.

The Referral Workflow

Six steps. No surprises.

  1. 01

    Submit the referral

Online form, fax, secure email, or direct coordinator call — whichever your office prefers.

  2. 02

    24-hour confirmation

Written acknowledgment with patient appointment offer or a clinical question if we need more information.

  3. 03

    Records request & intake

We contact the patient directly. We pull the records we need. You are not the middle layer.

  4. 04

    Comprehensive evaluation

A comprehensive new-patient visit with Dr. Johnson — within five business days for most conditions.

  5. 05

    Written report to your office

Findings, plan, recommendations, and follow-up timeline. Sent within 48 hours of the visit.

  6. 06

    Ongoing coordination

Periodic updates as treatment progresses. Direct line back to Dr. Johnson if your office has questions at any point.

Have a clinical question before you refer?

That is exactly what the provider-to-provider line is for. Most clinical questions get a direct answer from Dr. Johnson the same day — and many lead to a coordinated approach that may not even require a formal referral.
