Coverage and verification.

Pelvic floor conditions are medical, not cosmetic. They are covered as such by nearly every commercial plan, Medicare, and most Medicare Advantage plans. Here is what referring offices should know.

Accepted plans

We accept most major commercial plans, Medicare, and many Medicare Advantage plans. A current list is maintained on our patient insurance page. We will verify any specific plan during the referral intake.

Authorization process

Our office handles prior authorization directly with the patient's insurance, including for any planned procedure. We do not ask the referring office to obtain authorization. We will request a copy of the referring office's authorization documents only when the patient's plan requires the referring provider to initiate.

Self-pay and out-of-network

We see self-pay patients and patients whose plan is not in-network. Transparent pricing is provided in writing before any visit. The cost of a comprehensive new-patient evaluation, including in-office diagnostics, is competitive with what a patient would pay for a fragmented multi-visit workup elsewhere.

Documentation we provide

  • Closed-loop written report after every visit
  • Procedure documentation with CPT/ICD-10 codes as appropriate
  • Authorization-related letters of medical necessity, when needed
  • Copies of any in-office diagnostics performed
